New Patients

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. 

We are currently not accepting people who are currently outside of our catchment area.

Practice Area

Registration forms are available from our Reception Team by calling 01229 484141 or by using the online form.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. We also require two forms of ID when you hand the forms in, one photographic and one with a proof of address if possible.

All new patients between the ages of 40 and 74 will be offered a health check with a member of the healthcare team at some stage following registration to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.
