Travel Health Questionnaire

To help us offer the appropriate advice, please fill out the online form before coming to see the nurse.

Once completed, the nurse will look into your medical history and you will be contacted if any vaccinations are needed. 

If, for any reason, your trip is cancelled or you decide not to travel, please let the practice know as soon as possible.


It is important to complete your initial travel questionnaire as early as possible – at least 8 weeks before you travel.

Some are available on the NHS & others are only available privately. We are currently only able to provide the free NHS vaccines.

Many vaccines have to be ordered as they are not a stock vaccine and there are some supply issues with certain vaccines.

In practice it may take two to three months or more, depending on where you are intending to travel, for full courses of treatment to be completed to allow time for vaccines to become effective.  Please plan this as early as possible.

If travel vaccines are ordered on a private prescription they will incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS. Travel hubs and Private Travel vaccination companies can advise on these costs.
